General Instructions for Completing All Programs of Study

The Program of Study is the blueprint for your studies and should be submitted as soon as possible.
 
  1. Discuss with your advisor who must approve and sign form.
  2. Enter major—for all SLIS students, this is Library and Information Science.
  3. Leave Track or Specialization blank.
  4. Use SLIS for department abbreviation.
  5. Include prefix J and/or suffix, if course has either or both.
  6. Enter date completed or to be completed (ex. Fall '02, SSI '01)
  7.  Record grades for courses completed.
  8. Enter “Where Taken” only if work was done at an institution other that USC.
  9. Sign the form.
  10. Print your advisor's name in upper left-hand corner if form has not yet been signed by him/her.
  11. Return the form to the Student Services Manager (keep a photocopy if you wish).


NOTE:  Contact the Student Services Manager immediately if any change needs to be made in the program of study originally submitted.  All those who approved the original program must approve any change.  An adjustment form must be processed.
Adjustment Form – This form is used to make a change to a previously approved Program of Study.
 

1.  List the 36 semester hours of credit being used to earn your MLIS  degree.  We recommend listing only 36.  (School Media Certification  hours beyond the 36 for the degree should not be listed.)

2.  Joint Degree students must file a degree program with each department. These should be submitted to the Graduate School simultaneously. The MLIS degree program must have 36 hours listed.

3. SLIS 701, 707 and 705 are required. For graduation you also need to either pass the Computer Competency Exam or successfully complete SLIS 706. A protfolio is also required for graduation. If you exempted any of these, attach a note to the Student Services Manager.

4.  Include no more than 6 hours of Independent Study credits. 5.  Maximum age of courses is 6 years.

6.  Be sure appropriate documentation is on file for transferred credits. (Maximum 6 hours.)
 

Certificate of Graduate Study
Certificate of Graduate Study Program of Study

 
1.  List the 18 semester hours of credit being used to earn your Certificate.

2.  If you were eligible to use 6 hours from your MLIS degree, identify those hours with an asterisk and footnote.

3.  SLIS 794 is required ( 3 hours of credit).

4.  No Independent Study courses and no transfer credits are permitted in Certificate programs.

5.  Maximum age of courses is 4 years.


Specialist's Degree Program
Specialist in Library and Information Science Program of Study

 
1. List at least 30 semester hours of credit (or more) necessary for your degree. 3.  Be sure you list at least 18 hours of SLIS courses. 4. If you were required to take SLIS 705, Introduction to Research in Library and Information Science, list it (total of at least 33 hours). 5. If you were required to take any or all of the other core courses (SLIS 701, 702, 703, 704, 706), list them. If you exempted any of these, attach a note to the Student Services Manager. 6. Include no more than 9 hours of Independent Study (796) and Specialist Project Preparation (798). No more than 6 of these related to the research product/project. 7. Maximum age of courses is 6 years.

8. Be sure appropriate documentation is on file for transfer credits. (Maximum 9 hours only 6 of which could be taken prior to admission to the Specialist program)
