Immunizations

IMMUNIZATION POLICY

Beginning with the Spring 2005 term, all School of library and Information Science students currently enrolled, or beginning the MLIS, Specialist or Certificate degree programs, will not be exempted from the University immunization requirements.

This change in policy to require immunization of all students, includes the previously exempted cohort students.

The University of South Carolina requires all students born after 1956 to furnish proof of immunity for measles (rubeola) and German measles (rubella). There are additional requirements for international students. Students cannot register classes until an immunization clearance has been granted.

All students must complete the Immunization Requirement Form and mail or fax (803) 777-3955 the form and documentation of immunization to the Immunization Coordinator, Thomson Student Health Center , USC, Columbia SC 29208 prior to registration. You may also find the form at the health center link http://www/sa.sc.edu/thc/immunization1.htm.
